SG Enable - Hospital-to-Work (H2W) Programme

SG Enable – Supporting persons with acquired disabilities in their transition to employment.

The Hospital-to-Work (H2W) program is an employment program that support persons with acquired disabilities in their transition to work. H2W will help clients to identify personal goals that are achievable and manageable, and assist them in their endeavours

Key Features of H2W

  • Rehabilitation – Coordination of rehabilitation and social needs with a care professional

  • Re-integration – Vocational needs assessment & personal development and skills training

  • Employment – Job referrals and interviews & onsite job support

Who is eligible for Hospital-to-Work (H2W) Program?

  • Singapore Citizens or Singapore Permanent Residents who are motivated and with potential to work
  • Fulfils disability criteria* and requires other forms of social support

*Disabilities supported are: intellectual disability, physical disability, hearing impairment and visual impairment.

Staff from AGIS Medical participated in Project We Care – Garden Party @ Istana 2019. Companies and beneficiaries took a scenic walk around the Istana with then-President Halimah Yacob

AGIS Medical working closely with SMRT bus-captains on boarding of mobility aid such as mobility scooters and motorised wheelchairs.

Continuous improvement is important for AGIS Medical to explore opportunities to improve customer service, quality and reducing waste. AGIS Medical supports the National Healthcare Group initiatives for SMEs on the  Continuous Improvement Projects.

 

AGIS Medical co-organised the “Heritage Trail Using SMRT” project supported by the SG50 Celebration Fund. It aims to encourage Singaporeans to visit iconic places where they have fond memories with their families and friends by incorporating Singaporeans’ love for food and places of interest together. 

 

AGIS Medical supported the Ministry of Health in their Stay Active, Live Well program for seniors under their SG50 initiative

 

AGIS Medical supported the elderly with walking sticks/wheelchairs to assist them in moving around Singapore’s Gardens by the Bay.
